Connecting Clients to Care: Using PA Navigate for Referrals, Resources & Real Impact

Discover how PA Navigate is helping organizations across Pennsylvania strengthen community connections and simplify access to services. This introductory session will provide an overview of the platform’s history, purpose, and growing role in supporting coordinated care and resource navigation throughout the state.

Participants will receive a guided live demonstration of the platform, including how to search for community resources, connect individuals to programs and services, and understand the referral process from both the user and provider perspective. The session will also walk through what happens on the backend when referrals are received, how organizations can claim and manage their programs, and best practices for creating strong, clear, and effective program cards that improve visibility and engagement.

Christie Kilmer is a Community Engagement Manager at Community Action Association of Pennsylvania, where she works to strengthen partnerships and expand access to resources that support communities across the state. With over a decade of experience in nonprofit leadership, recruitment, and program development, Christie has a strong background in community outreach, volunteer engagement, and event planning.Previously, she served as a Membership Recruiter with Girl Scouts in the Heart of Pennsylvania and has held roles in education, administration, and leadership development. Christie holds a Master of Science in Organization and Management from Capella University and a Bachelor of Science in Business Administration from Keystone College. She is passionate about connecting people to opportunities and building collaborative solutions that create lasting impact.
